Job Description

Job Overview – Case Manager, Hospice (RN):
Join our client’s compassionate healthcare team in New Jersey as a dedicated Case Manager, Hospice (RN). In this role, you will manage a caseload of hospice patients, providing comprehensive care and support to patients and their families.

Location: Sussex County, NJ
Compensation: $53/hour
Schedule: 8:30 AM to 4:30 PM, Monday to Friday


Responsibilities as the Case Manager, Hospice (RN):

  • Patient Care: Perform ongoing physical and psychosocial assessments of hospice patients, ensuring adherence to the established plan of care.
  • Collaboration: Work with the interdisciplinary team to update nursing diagnoses and adjust care plans as needed.
  • Supervision: Instruct and supervise hospice aides and LPN staff, ensuring quality care delivery.
  • Communication: Coordinate with physicians, families, and other healthcare team members to report changes in patients’ conditions and identify needs.
  • Education: Provide patient and family teaching, using tools and handouts to support self-direction and independence.
  • Medication Administration: Prepare and administer medications, documenting all actions according to policy.
  • Documentation: Complete chart documentation within 48 hours, ensuring accurate records of vital signs, interventions, treatments, and patient responses.
  • Compliance: Maintain infection control practices and adhere to patient confidentiality standards.
Qualifications for the Case Manager, Hospice (RN):
  • Education: Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred.
  • License: Licensed to practice as a Registered Nurse in the State of New Jersey.
  • Experience: Minimum 1 year of adult, pediatric, or medical/surgical clinical experience (prior home care or hospice experience preferred).
  • Skills: Strong nursing assessment, documentation, and clinical skills.
  • Technical Skills: Proficiency in Microsoft applications (Word, Excel, Outlook) and electronic medical records.
  • Certifications: CPR certification preferred.
