Job Description
Overview:
We are looking for a compassionate Home Health RN to provide high-quality nursing care to patients in their homes.
Key Responsibilities:
- Conduct patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed.
- Monitor vital signs and document patient progress.
- Educate patients and families on care plans and self-care.
- Collaborate with healthcare teams for coordinated care.
- Provide specialized nursing services (e.g., wound care).
- Maintain accurate patient documentation in EHR.
- Participate in on-call duties as needed.
Qualifications:
- Current Maine RN license required.
- BSN preferred; associate degree accepted.
- Home health care experience preferred.
- Strong clinical assessment and communication skills.
- BLS certification required; ACLS preferred.
