Job Overview: Registered Nurse Assessor:
Join our client's team as a Registered Nurse Assessor in New York, NY. This hybrid role involves conducting home assessments and telehealth evaluations to support and assess patient needs. Location: New York, NY (Multiple Locations)
Compensation: $57-58/hour
Schedule: 2 Days Home Assessments & 3 Days Telehealth (Hybrid)
Responsibilities as the Registered Nurse Assessor:
- Assessment and Evaluation: Conduct initial in-home pre-evaluations and develop personalized care plans.
- Documentation: Review and document consumer medical information.
- Care Coordination: Discuss health care options with consumers and communicate with Call Center for managed care enrollment.
- Collaboration: Work with care team members to improve service delivery.
- Quality Review: Collect quality review data and maintain accurate records.
- Health Promotion: Identify opportunities for health promotion and illness prevention.
- Additional Duties: Perform other duties as assigned by management.
Qualifications for the Registered Nurse Assessor:
- Experience: Minimum 2 years of recent experience in a home setting (e.g., CHHA, LHCSA, MLTC).
- Preferred: Recent experience conducting UAS-NY assessments (within the last year) and experience caring for a geriatric population.
- Skills: Proficient in health status evaluations and UAS criteria, strong understanding of nursing principles, state and agency regulations, excellent organizational and communication abilities, and effective in a fast-paced, deadline-driven environment.
- Technical Proficiency: Proficiency with technology and electronic health records.
