Job Description
MedAdventures a division of Icon Medical Network is looking for a Registered Nurse (RN) Long Term Care to join our partner's full-time team in South Holland and Country club Hills, IL.
This is a DIRECT HIRE OPPORTUNITY
Key Responsibilities for the RN:
- Case identification/risk stratification to establish patients needs.
- Collaboration with the Transitional Care Team to reduce re-admissions/avoid inappropriate hospitalizations.
- Conducting and documenting a comprehensive assessment of barriers to care (medical, social, psychosocial, and literacy)Coordinating integrated care plan development, including collaboration with nursing staff, leadership, as well as the entire interdisciplinary team.
- Facilitation of developing appropriate referrals/consultations.
- Outcomes management and evaluation of the achievement of established goals within the plan of care.
- Ensures initial and annual follow up Model of Care training occurs for employees, providers and contracted personnel associated with ISNP program
Key Requirements for the RN:
- Current unrestricted RN license in the state of Illinois
- Minimum of 3 years clinical care experience. Skilled Nursing Facility/Long Term Care experience preferred.
- Case management experience preferred
- Strong nursing expertise in managing complex cases with minimal supervision.
- Knowledge of current standard medical practices and experience with Medicare/Medicaid.
- Valid license with vehicle and insurance
Compensation and Schedule for the RN:
- Target base salary 85,000 annually
- Monday-Friday, 8am-5pm working hours
- No Weekends, No Holidays, No Call
- Comprehensive benefits plan
- Will split time between two facilities (9 miles apart)
