Our structure is set up to provide Home Health Nurses multiple avenues for support that aren't available at other agencies – a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available every day to help with documentation questions and challenges), an in-office team of QI specialists, telehealth resources, clinical manager and in office triage support. As an RN Manager for Banner Home Care, you will support field clinicians by answering clinical questions, providing direction, education and coaching as needed. You will assist in collaborating and creating process for sharing necessary information with the Clinical Resource Leaders, Clinical Alignment Specialists, and Clinical Managers and assist with gathering necessary clinical information to provide care to patients prior to admission using evidence-based practice and innovation. You will work directly with leaders responsible for quality to ensure that documentation is completed appropriately and must have a complex and complete understanding of home health rules, regulations, and best practices. Because you will be working in a highly matrixed environment across all of Phoenix and with the regional back-office team, leadership experience is strongly preferred. Our RN Managers will also have a weekly productivity requirement. This position is a hybrid role and will be scheduled Monday – Friday.

POSITION SUMMARY
This position plans and provides clinical support to patients and staff. Demonstrates clinical expertise of the assigned area. Provides leadership in clinical, financial, and personnel management within the department.


CORE FUNCTION

1. Oversees the operations of the unit to ensure smooth and efficient patient care management in alignment with best practices and regulatory rules. Assures appropriate patient assignments and case management of patients. Completes daily rounding on patients and staff to ensure quality provision of care and assists with troubleshooting patient care issues.

2. Manages the unit to ensure patient and employee satisfaction while promoting safety and retention. Serves as a real-time resource and assists with clinical expertise for staff, patients, and families to ensure care delivery in alignment with regulatory agencies and requirements. Ensures effective use of resources, equipment, and supplies to deliver care in accordance with the care plan.

3. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.

4. Formulates care plans and oversees implementation of care plans for all patients in the assigned unit. Demonstrates clinical expertise in creation of a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.

5. Develops leadership skills among staff including communication, decision-making problem-solving/critical thinking, and employee engagement. Leads the development of staff and supports career advancement opportunities. Functions as a role model and encourages staff to participate in their own development.

6. Responsible for selection, orientation, on-boarding, and retention. Demonstrates leadership through coaching, performance evaluations, corrective actions, and development opportunities to create a culture of learning.

7. Provides daily operational resource management including staff, supplies, and equipment, and ensures optimal productivity for the department.

8. Supervises clinical unit staff to ensure excellent patient care. Participates in activities that foster management skills while enabling staff to care for patients, determine schedules, edit timecards, research budget issues and order supplies. Internal customers include patients, staff, and physicians. External customers include patient family.

MINIMUM QUALIFICATIONS

Requires a current RN license in state of practice. BLS required. Additional certification or continuing education may be required based on area of practice.

Must possess at least 1-2 years of clinical experience relevant to patient population. Demonstrates knowledge and skills of specialty area. Requires critical thinking, communication, influence, decision-making, analytical, and flexibility skills to make optimal decisions based on multiple variables and desired outcomes. Requires the ability to balance clinical skills with supervisory authority. Must have the ability to consider the ultimate impact of decisions on the unit. Must have the ability to practice skills according to department and professional standards and quality requirements. Requires New Leader Experience certification within one year. Employees working at Banner Home Care, Banner Behavioral Health Hospital, BTMC Behavioral or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
