Job Description

Job Overview – RN Nurse Care Manager (MLTC):
Step into a critical role with our client as an RN Nurse Care Manager (MLTC) in Brooklyn, NY. In this hybrid position, you will collaborate with members, families, and interdisciplinary teams to maintain independence, coordinate care, and ensure consistent healthcare delivery across the continuum. This role offers a blend of remote work and field-based responsibilities to provide comprehensive member support.

Compensation: $105,000 - $115,000/year
Location: Brooklyn, NY
Schedule: Monday to Friday (4 days remote, 1 day in the field)


Responsibilities as the RN Nurse Care Manager (MLTC):

  • Care Coordination: Collaborate with members, families, and caregivers to create and execute personalized care plans that promote independent living and optimal health outcomes.
  • Health Monitoring: Conduct ongoing assessments of members' needs and health status, ensuring seamless care across the continuum.
  • Service Authorization: Authorize covered services and coordinate care regardless of payer, ensuring compliance with program guidelines.
  • Interdisciplinary Collaboration: Communicate effectively with primary care practitioners, interdisciplinary teams, and other stakeholders to align care strategies.
  • Fieldwork: Provide in-person support across Brooklyn to address member needs and enhance care delivery.
Qualifications for the RN Nurse Care Manager (MLTC):
  • Education: Bachelor of Science in Nursing (BSN) required.
  • Licensure: Active and current NYS RN license and registration.
  • Experience: A minimum of 3 years of RN experience is required, along with Managed Long-Term Care (MLTC) experience and clinical expertise in geriatrics.
  • Skills: Bilingual proficiency required, with strong communication and organizational abilities.
  • Attributes: Compassionate, detail-oriented, and dedicated to improving member outcomes through collaborative care.
