Job Overview – Bilingual Russian RN (MLTC Nurse Care Manager):
Join our client as a Nurse Care Manager in Queens, NY. This hybrid role combines remote work with one field day per week, focusing on care coordination, advocacy, and facilitating optimal health outcomes for MLTC members. Fluency in Russian is required to effectively support and communicate with members. Your role will center on fostering independence and ensuring quality care for frail and disabled individuals who wish to remain in their homes.

Compensation: $105,000 - $115,000/year
Location: Queens, NY (Hybrid)
Schedule: 4 days remote, 1 day in field (Full-Time)


Responsibilities as the MLTC Nurse Care Manager:

  • Care Coordination: Provide comprehensive care coordination, including in-home assessments, service planning, facilitation, and advocacy.
  • Health Continuum Management: Ensure consistent care across the healthcare continuum by assessing and monitoring member needs and status.
  • Service Authorization: Authorize covered services and coordinate care regardless of payer sources.
  • Collaborative Support: Communicate effectively with members, families, caregivers, and interdisciplinary teams to deliver high-quality care.
  • Independent Living Advocacy: Support frail and disabled members in maintaining independent living arrangements for as long as possible.
Qualifications for the MLTC Nurse Care Manager:
  • Education: Bachelor of Science in Nursing (BSN) required.
  • Licensure: Active and current NYS RN License and registration required.
  • Experience: Minimum of 3 years of RN experience, with clinical expertise in geriatrics preferred (managed long-term care experience is a plus).
  • Skills: Strong care coordination and advocacy skills with the ability to work collaboratively in a team-oriented environment.
  • Language Requirement: Bilingual in Russian required to meet operational needs.
