Job Overview - Clinical Assessment Manager (RN):
Step into the role of Clinical Assessment Manager with our trusted healthcare client in Queens, NY. In this position, you’ll lead in-home and virtual assessments for PACE participants, ensuring accurate documentation of eligibility and care needs while collaborating with care teams to deliver seamless care.
Compensation: $110,000 - $120,000/year
Location: Queens, NY
Schedule: Monday to Friday, 8:30 AM to 5:30 PM
Responsibilities as the Clinical Assessment Manager (RN):
Step into the role of Clinical Assessment Manager with our trusted healthcare client in Queens, NY. In this position, you’ll lead in-home and virtual assessments for PACE participants, ensuring accurate documentation of eligibility and care needs while collaborating with care teams to deliver seamless care.

Responsibilities as the Clinical Assessment Manager (RN):
- Assessments: Conduct in-home or virtual assessments using UAS-NY to determine eligibility and care needs.
- Documentation: Review previous assessments and ensure accurate documentation in EMR, including narrative comments and supporting documentation.
- Clinical Consultation: Provide clinical consultation to Care Delivery teams and offer support during the intake process.
- Collaboration: Collaborate with Care Delivery, intake, and quality teams to improve processes and outcomes.
- Compliance: Ensure assessments, reports, and documentation are submitted timely, maintaining compliance with CMS/PACE guidelines.
- Education: Bachelor's degree in Nursing or a related health field preferred.
- Experience: 3-5 years in MLTC, LTHHCP, MAP, PACE, or managed care programs, and 5+ years in care management or related fields.
- Licensure: Registered Nurse (RN) with an active NY State License.
- Skills: Proficient in UAS-NY, Lenavi, and Microsoft Office; strong clinical management and communication skills.
- Other: Bilingual skills preferred, with the ability to work independently and meet deadlines in a fast-paced environment.
