Job Description

Case Manager RN - with a NY license* HOME HEALTH CERT Required**
40 hours per week guaranteed. Full benefits available day 1 on assignment.
Up to $3100 a wk
Requirements:
**NY license
***Certified Home Health Care Certificate Required***
***EPIC or Netsmart Required***
Schedule- Monday – Friday, 8 hour shifts, Day shift 8:00am-4:30pm, weekends per need
