Job Overview - Medical Case Manager (RN)
Deliver compassionate in-home nursing care as a Medical Case Manager (RN) with our client in Austin, TX. In this temporary role, you’ll assess patient needs, coordinate care across an interdisciplinary team, and ensure quality care and compliance with agency standards for patients of all ages.

Compensation: $46 - $50/hour
Location: Austin, TX
Schedule: Monday to Friday, 8:30 AM to 5:00 PM (Fieldwork Required)


Responsibilities as the Medical Case Manager (RN):
  • Assessments: Identify and evaluate patients’ physical, psychosocial, and environmental needs in collaboration with the interdisciplinary team.
  • Care Coordination: Deliver nursing care at the patient’s residence, aligning with the plan of care and in collaboration with other healthcare professionals.
  • Infection Control: Implement infection control and precautionary standards to prevent disease transmission during patient care.
  • Compliance: Ensure all cases meet third-party payor requirements and agency quality standards.
Qualifications for the Medical Case Manager (RN):
  • Education: Bachelor’s or Associate’s degree in Nursing from an accredited RN program.
  • Licensure: Current Texas RN license required.
  • Experience: Minimum of 2 years in a Clinical setting required (hospice experience preferred).
  • Skills: Skilled in effective communication with patients, families, and healthcare teams, with strong organizational and problem-solving abilities.
  • Transportation: Must have access to a reliable vehicle with active auto liability insurance for travel to patient locations.
