Direct Hire Registered Nurse $80,000 – $85,000 per year in South Holland, Illinois

Clinical Magnet a division of IMN Enterprises is looking for a Registered Nurse (RN) Long Term Care to join our partner's full-time team in South Holland and Country club Hills, IL.

This is a DIRECT HIRE OPPORTUNITY

Key Responsibilities:
• Case identification/risk stratification to establish patients needs.
• Collaboration with the Transitional Care Team to reduce re-admissions/avoid inappropriate hospitalizations.
• Conducting and documenting a comprehensive assessment of barriers to care (medical, social, psychosocial, and literacy)Coordinating integrated care plan development, including collaboration with nursing staff, leadership, as well as the entire interdisciplinary team.
• Facilitation of developing appropriate referrals/consultations.
• Outcomes management and evaluation of the achievement of established goals within the plan of care.
• Ensures initial and annual follow up Model of Care training occurs for employees, providers and contracted personnel associated with ISNP program

Key Requirements:
• Current unrestricted RN license in the state of Illinois
• Minimum of 3 years clinical care experience. Skilled Nursing Facility/Long Term Care experience preferred.
• Case management experience preferred
• Strong nursing expertise in managing complex cases with minimal supervision.
• Knowledge of current standard medical practices and experience with Medicare/Medicaid.
• Valid license with vehicle and insurance

Compensation and Schedule:
• Target base salary 85,000 annually
• Monday-Friday, 8am-5pm working hours
• No Weekends, No Holidays, No Call

• Will split time between two facilities (9 miles apart)
