Job Overview - Care Delivery Manager (RN):
Join our client's team as a Care Delivery Manager in Queens, NY, where you'll play a pivotal role in overseeing the development and execution of personalized care plans and ensuring high-quality care across nursing services.

Compensation: $53/hour
Location: Queens, NY
Schedule: 8:30 AM to 5:00 PM, Monday to Friday


Responsibilities as the Care Delivery Manager (RN):

  • Care Management: Oversee the development and execution of personalized care plans, ensuring they align with the health care needs and goals of all participants.
  • Team Coordination: Facilitate IDT meetings and Service Delivery Requests (SDR), ensuring effective communication and collaboration among team members.
  • On-Call Management: Serve as the manager on call during non-operational hours, managing follow-ups and documenting outcomes.
  • Clinical Oversight: Ensure the accuracy of clinical assessments, documentation, and compliance with regulatory standards.
  • Staff Management: Allocate staffing resources, evaluate staff performance, and assist with onboarding and professional development.
  • Regulatory Compliance: Ensure all activities adhere to federal and state regulations, particularly in documentation, care management, and incident reporting.
Qualifications for the Care Delivery Manager (RN):
  • Education: Bachelor of Science in Nursing (BSN) required.
  • Experience: 2+ years in Management within Home Health, Acute Care, or similar settings (experience with frail/elderly populations preferred).
  • Licensing: Active RN license and registration in New York State.
  • Additional: Experience in customer service and managed long-term care insurance is a plus.
