Your Day-to-Day

• Home and facility Visits: Travel to home-bound and skilled nursing facility patients to perform wound care rounds, collaborate with healthcare teams and deliver essential patient care. Early morning starts may be required.
• Wound Care Expertise: Provide expert care for chronic, surgical, and dermatologic wounds. Duties include bedside debridement, dressing changes, and G-tube care (with training available if needed).
• Detailed Documentation: Keep accurate and timely records in our electronic medical record (EMR) system to ensure the highest level of care.

Who We’re Looking For

• Experience: Wound Care Certification and at least 2 years of experience are preferred.
• Independent & Motivated: You’ll need to be self-driven, an excellent communicator, and able to work autonomously.
• Licensure: Must have an active Texas NP, DO, MD, or PA license. Certified Nurse Practitioners are a plus!
