(Archived) Primary Care NP or PA

Specialty accepted: Primary Care NP or PA
Start date: As soon as credentialed – Ongoing (6-month minimum commitment, 1 year preferred)
Schedule: Monday to Friday, 8:00am – 5:00pm (8-hour guarantee)
Practice setting: Home Health
Patient load: 3-4 patients per day (up to 6 if in close proximity)
Ages: Adult patients
Procedures required: Primary Care, Wellness Exams, Chronic disease, population health, Kidney care
Requirements: BLS, DEA, active state license, board certification
Other: Long-term assignment with flexibility for 4–5 shifts per week; opportunity to support a growing home health model focused on CKD/ESRD patients; credentialing takes time, but client is highly motivated to maintain and extend providers.

Benefits: Malpractice insurance coverage; weekly electronic pay; mileage reimbursement; earn $1,000 per referral.
