(Archived) Primary Care NP or PA

Specialty accepted: Internal Medicine or Geriatric Home Health Nurse Practitioner or Physician Assistant
Start date: ASAC – ongoing (6-month minimum commitment, start as soon as credentialed.
Schedule: Monday to Friday 8a–5p (8-hour guarantee)
Practice setting: Home Health
Patient load: 3–4 patients per day (up to 6 if in close proximity)
Ages: Adults
Procedures required: Primary Care, Wellness Exams, Chronic Disease, Population Health, and Kidney Care
Requirements: BLS, DEA, active state license, board certified
Other: Orientation includes a 3-day paid trip to Brentwood, TN. Mileage reimbursed at $0.44/mile. Providers work closely with RN and LCSW teams in the field. Must be comfortable with home-based care. Kidney care experience preferred. Multiple openings available with long-term potential.
