Job Description
A growing group in the Trenton New Jersey area has an opening for their House Calls Program.
- Board-Certified Nurse Practitioner
- NJ license/DEA in hand
- Seeking candidates with 1+ years of Home Health or Medicare populations
- NP's make 6-7 visits daily in a defined territory
- Will collaborate with Home Health, Specialists, Social Workers, Hospitals, SNF's, Etc.
- Support staff available
- Ages seen are mostly 55+
