Schedule: Standard full-time hours

Experience: Prefer 3 years of related experience, but new graduates will be considered

Patient Population: Patients on anticoagulation therapy

In this Role:

As an Anticoagulation Clinician at a healthcare facility in Chelan County, WA, you will be the anticoagulation medication expert on the integrated care team. Your responsibilities will include performing clinical management of patients on anticoagulation therapy, adjusting medication dosages, and developing and coordinating perioperative and procedural plans. You will counsel patients and their family members on a full range of issues related to anticoagulation therapy and provide patient and family education. Additionally, you will coordinate communication with patients and other healthcare providers regarding medication therapy and associated disease states. You will report to the Pharmacy Manager and be a vital resource to your team.
