We are hiring an experienced Digestive Disease Physician Assistant/ Nurse Practitioner to work with a high profile Esophageal Disease multidisciplinary team.
The Advanced Practice Provider - Esophageal Diseases will expand the capacity to treat patients in need of Reflux/GERD, Barrett’s Esophagus, Achalasia, various Esophageal Cancers and assist in the delivery of critical diagnostic and therapeutic procedures such as pH impedance and manometry in addition to conducting return patient visits in clinic(s).
Responsibilities:
Performs duties as an Advanced Practice Provider, providing comprehensive care to patients in the outpatient Digestive Diseases - Esophageal and MIS Clinics.
Works collaboratively within a multidisciplinary team, using advanced clinical skills to manage a caseload of patients.
Performs the assessment, diagnosis, initiation of delegated medical treatments, independent provider interventions, and evaluation of care.
Provides a model of care that is shaped by theory, expanded by science, research and reflective practice, and is identified by a holistic and comprehensive approach to the patients.
Provides support and follow up to digestive diseases - Esophageal and MIS patients in the outpatient setting.
May be trained and may be responsible to perform certain diagnostic and/or therapeutic procedures in an outpatient setting and or ambulatory surgical center setting, including manometry and pH studies.
Demonstrates ability to appropriately perform telephone triage regarding patient concerns and complaints, with appropriate communication.
Conducts comprehensive or episodic health assessment of patients, gathering pertinent information in a systematic manner from varied sources.
Differentiates normal and abnormal variations in organ systems.
Analyzes all sources of data, establishes a medical diagnosis: initiates and evaluates diagnostic studies, monitors therapeutic procedures and the patient's response to therapy.
Formulates a plan of care in collaboration with patients, family members, physician, and other members of the health care team.
Monitors Esophageal diseases and other comorbid conditions and incorporates assessment findings into plan of care.
Provides anticipatory guidance to patients and their families about the course of therapeutic options available.
Performs, interprets, monitors and communicates results of diagnostic tests.
Counsels patient concerning lifestyle modifications, medication regimens, side effects and interactions using an appropriate psychosocial communication level.
Recognizes an emergency situation and initiates appropriate emergency care.
Identifies complications requiring collaboration and consultation, initiating appropriate action.
Promotes, has knowledge of, and utilizes appropriate patient care protocols that relate to the physiological and psychological needs of various digestive diseases with a clear emphasis on Esophageal, Reflux patients.
Schedules follow-up visits to appropriately monitor patients, disease progression, response to therapy, post-visit or procedure (EGDs), or surgery.
Establishes appropriate priorities of care.
Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.
Performs other related work as needed.
