Job Summary
We are seeking a skilled Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our healthcare team in Elmira, NY. As an Advanced Healthcare Practitioner, you will be providing comprehensive primary care services to patients of all ages in a collaborative, patient-centered environment. This role offers the opportunity to be part of a multidisciplinary team delivering high-quality care to individuals in the community.
Key Responsibilities:

Provide primary care services to patients across the lifespan, including conducting physical exams, ordering and interpreting diagnostic tests, diagnosing and treating medical conditions, and prescribing medications

Perform routine health screenings, preventive care, and immunizations

Develop and manage individualized treatment plans for patients

Educate patients on health promotion, disease prevention, and management of chronic conditions

Collaborate with physicians and other healthcare providers to ensure comprehensive care

Maintain accurate and up-to-date patient records and documentation

Participate in quality improvement initiatives to enhance patient care

Adhere to clinical guidelines, policies, and safety protocols

Education and Experience Requirements:

Graduate from an accredited Nurse Practitioner or Physician Assistant program

Active New York State NP or PA License

Minimum of 2 years of experience in primary care or family medicine preferred

Current CPR certification required

Strong clinical skills and ability to make evidence-based decisions

Ability to work both independently and as part of a healthcare team
