Job Summary
We are seeking an experienced Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Colebrook, NH. This position involves providing high-quality primary care to individuals and families in a rural health clinic setting. As part of a dedicated healthcare team, you will deliver comprehensive health services, including preventative care, diagnosis, treatment, and management of common and complex health issues.
Key Responsibilities:

Provide comprehensive care to patients of all ages within a family medicine practice

Perform routine physical exams, preventive health screenings, and immunizations

Diagnose and treat acute and chronic medical conditions

Prescribe medications and manage treatment plans

Collaborate with other healthcare providers to coordinate patient care

Educate patients on health maintenance and disease prevention

Document all patient encounters in an accurate and timely manner

Participate in patient care planning and quality improvement initiatives

Adhere to clinic policies, state regulations, and industry standards

Provide compassionate, patient-centered care to a diverse community in a rural setting
Professional Expectations:

Work collaboratively with physicians, other APPs, and clinical staff

Demonstrate a commitment to patient safety and quality care

Maintain professionalism and confidentiality in all interactions

Provide leadership and mentorship to less experienced staff when needed

Display strong communication skills and the ability to connect with patients and the community
Education and Experience Requirements:

Graduate from an accredited Nurse Practitioner or Physician Assistant program
-
Active State of New Hampshire NP or PA License
-
Minimum of 2 years of experience in Family Medicine or Primary Care

Experience in rural healthcare settings preferred

Current CPR certification
-
Strong clinical judgment and the ability to make evidence-based decisions

Ability to work independently and as part of a collaborative team
