Job Description
Position Overview:
Role: Family Nurse Practitioner (FNP) or Physician Assistant (PA) – HOPE Program
Schedule: Full-time
Location: Various settings, including a mobile outreach van, homeless encampments, medical respite, hotels, shelters, and on the streets
Responsibilities:
- Work closely with the Medical Director of HOPE to provide healthcare services to unsheltered individuals
- Serve as the primary care provider for patients experiencing homelessness or housing instability
- Identify system gaps and contribute to program quality improvement
- Independently assess and treat acute and chronic conditions in both episodic and primary care settings
- Provide care in specialty areas, including:
- HIV, hepatitis C, sexual and reproductive health
- Transgender health and family planning
- Behavioral health, addiction medicine, and urgent care
- Deliver full-scope care across various environments, including streets, encampments, temporary and permanent housing, motels, and clinical settings
- Adapt to diverse care delivery models for patients experiencing homelessness
- Supervise nursing staff, medical residents, interns, and NP students during clinical placements
- Must have an active California license
Compensation & Benefits:
- Salary: $113,100 – $146,900 per year
