Job Description

Position Overview:

Role: Family Nurse Practitioner (FNP) or Physician Assistant (PA) – HOPE Program
Schedule: Full-time
Location: Various settings, including a mobile outreach van, homeless encampments, medical respite, hotels, shelters, and on the streets

Responsibilities:

Work closely with the Medical Director of HOPE to provide healthcare services to unsheltered individuals
Serve as the primary care provider for patients experiencing homelessness or housing instability
Identify system gaps and contribute to program quality improvement
Independently assess and treat acute and chronic conditions in both episodic and primary care settings
Provide care in specialty areas, including:
- HIV, hepatitis C, sexual and reproductive health
- Transgender health and family planning
- Behavioral health, addiction medicine, and urgent care
Deliver full-scope care across various environments, including streets, encampments, temporary and permanent housing, motels, and clinical settings
Adapt to diverse care delivery models for patients experiencing homelessness
Supervise nursing staff, medical residents, interns, and NP students during clinical placements
Must have an active California license

Compensation & Benefits:

Salary: $113,100 – $146,900 per year
