Job Description

Great opportunity in the Northern Orange County area of CA for a Nurse Practitioner to perform annual wellness exams working for a private group.

Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)
2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus
Perform an annual wellness exam for senior Medicare patients
Collaborate with Primary Care team to ensure completion of holistic patient care
Combination of home health and telehealth - 30 mile radius
If you are not located in the Northern Orange County area you must be comfortable commuting to the Northern Orange County area and then an additional 30+ miles within the Northern Orange County area
Must have active CA license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have AWV exp.
Mon-Fri 8a-5p
8-10 patients per day
