- Work Dates Needed: 11/4-8, 11/11-15, 11/18-22, 11/25-27, 11/29, 12/2-6, 12/9-13, 12/16-20, 12/23-24, 12/26-27, 12/30, 12/31
- Preferred Schedule: Mon, Wed and Fri 7a - 4p; Tues and Thurs 8a - 5p
- Worksite Setting: Clinic
- Scope of Work: Family Practice, Primary care experience and Work Comp Knowledge
- Licenses, Certifications, Requirements: Active IN License, BLS, DEA, and 2 years of family practice experience.
- EMR: EPIC or MD-Scripts
- Pay Rate Preferred: $75-80/hr
