Your New Role:

• Position: Nurse Practitioner - Mobile Health Wellness Assessor

• Job Duration: Locums

• Setting: Mobile healthcare, traveling from home to home

• Patient Population: Primarily Medicare patients with some Medicaid, focused on annual health and wellness assessments

• Assignment Dates: Start ASAP, schedule based on provider availability (minimum 2 days per week)

• EMR: Tablet provided, LUMEN Charting and Scheduling platform

• Schedule: Flexible schedule with assessments in the comfort of patients' homes, ideal for candidates who appreciate a caring approach to patient interaction.

• Compensation: $100 per assessment with a travel stipend—$60/day for up to 120 miles traveled, $300/day for over 120 miles traveled.

Requirements:

• State License: Active Arizona license required

• Certifications: Nurse Practitioner; Must have appropriate licensure and certifications

• Experience: Minimum 3 months of experience as a Nurse Practitioner; ideal candidates will have a strong appreciation for this patient population with excellent bedside manner

In this Role:

As a Nurse Practitioner, you’ll perform annual wellness assessments for Medicare patients, traveling to their homes in Mohave County, AZ. Each assessment takes roughly 45-60 minutes, and you'll see approximately six patients per day, with a minimum of two shifts per week. This role requires a provider who is compassionate and enjoys working independently while delivering quality care. You'll report any significant findings to the patient's Primary Care Physician and assist in coordinating emergency care if needed.
