Your New Role:

• Position: Nurse Practitioner - Home Health Assessment

• Job Duration: Locums

• Setting: Mobile Healthcare, In-Home Assessments

• Patient Population: Primarily Medicare, some Medicaid

• Assignment Dates: Ongoing, with shifts available 7 days a week

• EHR: Tablet provided – LUMEN for charting and scheduling

Requirements:

• State License: Ohio

• Certifications: Nurse Practitioner license

• Experience: Minimum of 3 months Nurse Practitioner experience

• Two Days per Week: Flexibility with days

In this Role:

As a Nurse Practitioner in this locums position, you will conduct in-home health and wellness assessments for Medicare and occasionally Medicaid patients in the patient’s home in Central Ohio area. Your role will involve asking questions about the patient's health, environment, and overall well-being. No prescribing or bloodwork required. Assessments are roughly 45-60 minutes. Schedules are flexible, allowing you to set your availability at the beginning of each month.
