Specialty: NP Family Practice
Start Date: 9/15/24
Contract Length: Min of 3 months with option to extend
Shift Details: M-F 9a-6p
Schedule: Full time

Work Details:
· Amount of coverage needed per month: Full Time
· Patients per day: 15-20, outpatient
· Patient Population/Age: all ages, mainly adults
· Department Description: outpatient clinic
· EMR: Athena Practice

Practice Setting:
• Clinic

Requirements:
· Board Certification: Board Certified
· License required: AZ license
· DEA: AZ DEA
· CSR: N/A
· Certifications required: Covid Vaccinated
· Minimum experience required: 3 years

Credentialing Timeframe: ~30 days
