LOCATION: Middleton, WI (Next to Madison)

JOB TYPE: 1099 Locums Contract

Schedule:

· ASAP-8/15

· M-F 8am-4:30pm


DESCRIPTION:

· Direct primary care for the employees and dependents; well care, acute and chronic care for individuals 2+

· Site also has lab, behavioral health and physical therapy.

REQUIRED:

· WI License

· BLS

COMPENSATION:

· Competitive Market Rate

· Malpractice insurance provided
