Dates: 6/26-27, 7/12, 7/19, 7/24, 7/26,7/31, 8/2, 8/7, 8/9, 8/14, 8/16, 8/21, 8/23, 8/28, 8/30 Mondays & Wednesdays: M 6a-4p, W 8a-6p
Cambridge City, IN: NP or PA
Dates: 6/27, 8/1, 8/8, 8/15, 8/22, 8/29 Tuesday 10a-6p
Yorktown, IN: NP or PA
Dates: 6/29, 7/6,7/13, 7/27, 8/3, 8/10, 8/17, 8/24, 8/31 Thursdays: Th 9a-5p
Peru, IN: NP or PA
Dates: 7/12-13, 7/31, 8/2, 8/8-9, 8/14, 8/16, 8/21-25, 8/28, 8/30 shift times: Mon 9a-6p, Tues 6a-1p, Wed 9a-6p, Thurs 6a-1p
Outpatient/Primary Care for Employees and Family
Patients: Avg. 10-15 PPD, ages 2 and up
Procedures: Suturing preferred, but not required; mostly acute care
Requirements:
Active license in IN
Minimum of 3 years of outpatient, clinical experience (as an NP, PA, Physician) in a Primary Care and/or Urgent Care setting.
Must be comfortable working independently
DEA required
BLS req'd
BC req'd
Privileges: Approximately 2-3 weeks
Local candidates preferred due to PRN
