Req#63327 11/21-2/3 Mon-Wed & Fri each week: M/W 5:15a-4:15p, Tu 6:45a-5:15p, F 7:45a-3:15p

Outpatient - Employee Health Clinic

Primary Care with some OccMed

Ages 2 and up - seeing the employees and their dependents

Average 2 to 3 patients per hour

Prefer local candidates that can commute

Requirements:

Must have KY license with full practice authority

DEA

BLS

At least four years' of experience
