Shifts: 9/19 - 4/30/23 SEE NOTES BELOW
Hours 8am to 5pm
Duties: Primary Care, Urgent Care.
Requirements:
IL license, BLS and DEA is required.
**First 3 weeks will be 40 hours weekly. Subsequent weeks will be 8 hours every Wednesday and including the following dates: 11/23 and 12/28. Additional coverage could be added if provider available. Beginning mid to late Jan full-time M-F 40 hours weekly. Hours as below: Monday-8-5 Tuesday-10-7 Wednesday-8-5 Thursday-8-5 Friday-8-5

Shifts: Tues, Wed 8:30-5pm, Thurs 10-7pm, some Saturdays
EMR EPIC
Ages 2 +
REQUIREMENTS
Active IL license
Active DEA
Certifications BLS
Minimum 1-2 years of FNP experience
Highly prefer DOT cert and EPIC experience
