Job Overview - Medical Case Manager (LPN)
Join our healthcare client’s team as a Medical Case Manager in Trenton, NJ, where you’ll provide compassionate, individualized care across multiple New Jersey counties. This temporary role involves traveling to clients’ homes to ensure exceptional care quality.

Compensation: $40/hour
Location: Trenton, NJ
Schedule: Monday to Friday, 8:30 AM to 5:00 PM (Fieldwork Required)


Responsibilities as the Medical Case Manager (LPN):
  • Home Visits: Travel within designated counties to conduct in-home assessments and monitor client health.
  • Clinical Oversight: Perform physical assessments, review home environments, and examine medical binders, ensuring all care needs are met and reporting any abnormalities.
  • Documentation: Record notes and assessments in the Electronic Health Record (EHR) system.
  • Collaboration: Communicate with physicians and healthcare professionals to advocate for and meet client needs.
  • Field Work: Work independently with remote supervision, managing a flexible schedule in the field.
Qualifications for the Medical Case Manager (LPN):
  • Education: Bachelor of Science in Nursing (BSN) from an accredited program required.
  • Licensure: Active LPN license in New Jersey required.
  • Experience: 2-3 years as an LPN, preferably working with individuals with intellectual and developmental disabilities (I/DD) (home healthcare experience is a plus).
  • Driving Requirements: Valid driver’s license with a clean record, with ability to travel between client locations.
  • Technical Skills: Proficient with computers and documentation systems.
  • Communication: Strong English skills for effective client interactions and team collaboration.
