Job Overview - LPN, Home Care:
Our healthcare client is seeking a dedicated Licensed Practical Nurse (LPN) for Home Care to join their team in New Jersey. This vital role involves providing clinical support, adhering to physician orders, and assisting with personal hygiene and daily activities. The ideal candidate will be skilled in patient reporting, equipment handling, and patient education.

Location: New Jersey, NJ
Compensation: $44/hour
Schedule: 8:30 AM to 5:00 PM, Monday to Friday


Responsibilities as the LPN, Home Care:
  • Clinical Support: Perform treatments and procedures according to standards and policies, following the plan of care directed by RN or clinical supervisors.
  • Physician Compliance: Adhere to physician orders and comply with all relevant regulations and policies.
  • Patient Reporting: Monitor patient conditions, record vital signs, and report changes promptly.
  • Patient Assistance: Assist with personal hygiene, nutrition, daily activities, and medical treatments.
  • Equipment Handling: Properly use and maintain technical equipment and supplies.
  • Patient Education: Support patient teaching and discharge planning.
  • Emergent Situations: Participate in emergency situations, assisting and reassuring patients and families.
  • Data Collection and Reporting: Complete data collection, care plans, and interventions accurately, ensuring timely completion of assignments.
Qualifications for the LPN, Home Care:
  • Education: Graduate of a LPN program.
  • License: Current and valid LPN license in New Jersey.
  • Certifications: BLS/CPR Certification by the American Heart Association.
  • Experience: Certified Home Care experience preferred; clinical experience with patient care required.
  • Technical Skills: Epic experience preferred.
  • Language Skills: Fluency in Spanish preferred.
  • Skills and Attributes: Quick learner, adaptable, accountable, dedicated to ongoing education, excellent customer service, and committed to high-quality care.
