Salary: $28-$35 / per hour

Position Description:

The Licensed Vocational Nurse (LVN) assumes responsibility and accountability for the application of the nursing process and the delivery of patient/client care and/or contracted services under the supervision of a registered nurse or licensed designee. The LVN provides service in accordance with Maxim philosophy, standards and policies.

Essential Duties and Responsibilities:

  1. Utilizes the nursing process to plan, implement, and evaluate patient/client care and/orcontracted services
  2. Demonstrates on each assignment the provision of skilled nursing care rendered in accordancewith prescribed orders
  3. Identifies and documents changes in patient’s both physiologic and psychosocial condition
  4. Notifies Director/Supervisor if applicable on the following;
  5. Physician notification to obtain necessary orders for intervention(s) per stateregulations
  6. Changes in patient/client condition
  7. Adheres to Maxim's policies and procedures and applicable protocols
  8. Identifies and prioritizes actions based upon patient/client care requirements
  9. Assures patient safety at all times and recipient of services provided
  10. Verifies that documentation reflects patient/client collaboration, participation in plan of care,expected patient/client outcomes, and selected nursing interventions
  11. Performs interventions, treatments, and medication administration, according to identifiedpriorities, plan of care, facility policies and patient/client care outcome standards as applicable.

Monitors and documents effectiveness of interventions, treatments, and medications

  1. Evaluates patient/client and provides a safe, comfortable and therapeutic patient/clientenvironment
  2. Monitors the safety and effectiveness of the environment and equipment including universalprecautions
  3. Participates in activities to facilitate good interpersonal communications
  4. Communicates observations and significant changes in patient/client status to the Director of

Clinical Services and/or Clinical Supervisor /Case Manager or licensed designee

  1. Documents all physician collaboration/coordination of care in the medical record andcommunicates this information as needed
  2. Attends case conferences, team meetings, education in-services as required
  3. Supports professional practice by observing the following;
  4. Nurse Practice Act
  5. Standards of Nursing Practice
  6. Standards of Patient Care
  7. Standards of Accrediting Body (ACHC, Joint Commission, etc.)
  8. State and Local Laws
  9. Patient Bill of Rights
  10. Maxim Healthcare, Nursing, and Client’s Hospital Policies
  11. Performs other duties as assigned

Minimum Requirements:

  1. Graduate of an accredited vocational nursing program
  2. Licensed Vocational Nurse in specific state
  3. Complies with accepted professional standards and practice
  4. At least one year prior professional nursing experience, preferred
  5. Willingness to participate in skills advancement training or novice nurse program if applicable
  6. Current CPR as applicable, for direct patient care
  7. PPD or chest x-ray if applicable
  8. Successful completion of new hire training as applicable to job site
  9. Participation and completion of Maxim’s Competency Program
  10. Strong verbal and written communication skills
  11. Current Health Certificate (per facility contract or state regulation)
  12. Must meet all federal, state, and local requirements

Compliance and Ethics Expectations:

  • Participates and successfully completes the company’s compliance program requirements andadheres to the Code of Conduct, Company policies, and applicable federal and staterequirements
  • Sets an example for other employees regarding how the Company’s Code of Conduct and

Compliance Program is applied and observed every day when dealing with customers, business operations, or other team members

  • Reports potential violations of company policy, Code of Conduct, and/or applicable laws andregulations to the company hotline, through the chain of command, to the Compliance and

Ethics Department, or through other channels made available by the company for reporting potential violations

  • Promotes an environment in which other employees are encouraged to report potentialviolations
  • As appropriate, provides input and suggestions regarding areas in which policies, procedures,workflows, and/or controls can be improved to enhance compliance
