Job Overview - LPN Care Manager:
Our healthcare client is seeking a Licensed Practical Nurse (LPN) to manage care for individuals in a residential setting. This full-time, onsite role involves promoting health and well-being by working with a team of healthcare professionals and offering support to both the individuals and their families.
Location: Brooklyn, NY
Compensation: $40/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the LPN Care Manager:
- Health Assessment: Identify health needs through home visits and direct interactions.
- Medical Home Establishment: Create a medical home for adolescents.
- Liaison and Support: Act as a liaison between providers and offer emotional support to clients and families.
- Problem Resolution: Resolve client issues with a multidisciplinary approach and maintain accurate documentation.
- Communication and Collaboration: Share health information with clients and team members, fostering teamwork.
- Medication Administration: Administer medications and monitor responses.
- Sanctuary Model: Implement and practice Sanctuary Model tools and framework.
- Education: Graduate of an accredited practical nursing program.
- Licensure: NYS LPN Licensure and CPR Certification.
- Skills: Strong nursing principles, excellent communication skills, and collaborative abilities.
- Experience: CPR Certification for infants, adolescents, and adults.
